- Understand the process when a caregiver initiates a care team and invites the patient, other caregivers, and/or informal caregivers.
- Identify what triggers this process.
- Determine which subsystems are involved.
- Define what needs to happen to map identities.
- Clarify what actions are required on the matrix side.
