Retry with backoff for pg pool ENOTFOUND during restarts

Seen once during force-recreate: getaddrinfo ENOTFOUND db. Add a 1-2 attempt retry on ENOTFOUND/ECONNREFUSED in src/lib/db/index.ts so mid-restart requests self-heal.
