Add code attributes to web exception events

I want to be able to see what file is causing exception events to be created when using the instrumentation-web-exception package.

Describe the solution you'd like to see

As discussed in the original proposal, we should have on the event attributes describing the source code generating the exceptions.

Manually adding the attributes
