Only 'Health Institution' dropdown is showing for 'Event details' section in Advanced search for death.

Bug description:
In advanced search for death, only 'health institution' dropdown is available under Event details section.

Steps to reproduce:

1. Navigate to advanced search death tab.
2. Observe the Event details section.

Actual result:
Only 'Health Institution' dropdown is showing for 'Event details' section in Advanced search for death.

Expected result:
Under event details sections, there should a dropdown for 'Place of death' with options -

• Health Institution
• Residential address
• Other

When Health institution is chosen, health institution dropdown should appear.
When Residential address is chosen, respective fields should appear. (country, province, district, town, residential area, street, number, postcode/zip).
