Bug Title:
Data Discrepancies Identified in informant's details for death

Steps to produce:

1. Search for records having tracking ID: D2H4KYR
2. View the record and observe informant's details

Actual Result:

1. Relationship to informant is missing when informant type is "Other"
2. Shows invalid date when informant's age was provided
3. National ID becomes required though it was not mandatory
4. Email becomes required though it was not mandatory

Expected Result:

1. Relationship to informant should display according to provided data
2. Informant's age should be displayed according to provided data
3. National ID is optional , so should not show required
4. Email is optional , so should not show required

Required fields are a form mismatch.
Age will be fixed when the new age component is implemented.

@tumbledwyer All other issues are fixed. but it shows invalid age though it's valid (56 years)

@tumbledwyer provide data was age, but after migration it's showing Date of birth. Expecting Age of informant instead of date of birth.
