Bug Title:
Supporting documents issues for death record

Steps to produce:

1. Search for record having tracking ID: DAY7ONO, DMHG3JF, D2H4KYR
2. /View records and observe supporting documents section

Actual Result:
On left side, proof of death is appearing twice with proof of deceased's ID. On right side, proof of death is missing and it shows deceased's ID.

Expected result:
All supporting documents should appear according to provided data.

Migration fixed.

Form configuration bug still exists. Proof of death and proof of deceased share a label id.
