Create additional forms

🚀 Requirements

As a user
I want to be able to create any of the available forms

As of the writing of this ticket, we do not have all the form field data.
Once that data is available a decision needs to be made whether we use one form and differentiate by type, or use separate forms for each event.

In the case we create one form, update the existing UI and endpoints to take the generic form.
In the case of multiple forms, create new tickets for each form.

Please update this ticket accordingly once the form definitions are available.
