Create the Death form frontend

We have recieve the form configuration for the Death from https://docs.google.com/spreadsheets/d/1G0bzf4kIj_2oXOJpvGgvr0hVZECDdNnL/edit?gid=1846692854#gid=1846692854

So we'll need to build the Death form UI up to the point that it submits the form values to the backend(even though the backend is not ready yet)

As a user I want to open the form by clicking on the Birth from the capture and see all the form fields available with the submit button sending the request to the backend

Other tasks:

• When the agent clicks on the Exact Date of Birth unknown checkbox the Date of Birth field should disappear and an Age in years field should be displayed for the user
