Submit Form: Bad Phone Clears Start and End Times

Issue:
If you enter a bad phone number (for example: 2210-555-55555) all details remain except for Start/End times.

Expected Behavior:
The Start/End times should stay on the form when there is an error with another field.

Steps to Recreate:
Go to http://localhost:8000/submit/

Enter details for the form and a bad phone number:

2210-555-55555

Click "Submit"

You will see the fields have cleared out.
