Add the Side-effects task for male

• Side-effects are the first task in the family planning follow-up visit. It is a required task for the CHW to record before the visit can be submitted. It always appears in the family planning visit.
• See row 3 in CHW 3 - FP - Follow up visit - Male in the data dictionary
• Native form widget

Once task is completed:

• If condom_side_effects != "None" and action_taken = "No action taken", task is yellow, otherwise it is green.
• [x The text under the completed task should be:
  (1st row) Side-effect: {condom_side_effects} (depending on the {fp_method_accepted}; the list should be comma separated if multiple side-effects are selected)
  (2nd row) Action: {action_taken}
