Add `switch_id` to mgd metrics and ddm metrics

The mgd and ddm daemons running in the switch zone currently do not report `switch_id`, only `sled_id` in metrics. It would be helpful for consumers of this data to have the `switch_id`, as the relationship between `switch_id` and `sled_id` can change over time.
