chore: remove internal planning docs, prompts, and specs from public repo

Removed:
- specs/prompts/ (7 LLM system prompts)
- PROGRAM.md (behavioral spec)
- ADR-001-tools-over-pipeline.md (architecture decision record)
- docs/ROADMAP.md, FEATURE-STATUS.md, TESTING-LOG.md (internal planning)
- docs/INSTALL-OPENCLAW.md, INSTALL-OPENCLAW-MIGRATION.md (internal migration)

Updated references in README, QUICKSTART, HOW-MEMORY-WORKS, INSTALL-HERMES,
and skill setup docs to point to remaining public docs.
