Add documentation

There is some documentation within, but this includes:

• Proper PHPDoc block styling
• Actual documentation usage documentation (in wiki, initially)

A decent amount of documentation has been added to the wiki. But, more could exist. Will leave this open until at least two more additions are made. :)
