Add a new version of ApplicationTraceIndex table

Create a new ApplicationTraceIndex table used in scatter chart, transaction list, and filtered server map.

Main changes:

• Increase maximum application name length to 254 characters
• Include serviceTypeCode in the row key
• Use double hashing for row key distribution
• Reserve 4 bytes in the row key for application grouping

Additional changes to qualifier and values

• use spanId as qualifier instead of transcationId
• Do not use variable-length encoding in elapsedTime
