Update Grafana instructions to include setting the Pixie Cloud address step

In version 0.0.2 of the Grafana plugin, users have the option to provide their own address for Pixie Cloud if it isn't hosted at withpixie.ai. We should update the instructions to include that step, because it wasn't present in version 0.0.1.
