Add 'Default Hiera Config' template item (enhancement for puppet-module-base)

Based on the research and decision making in puppetlabs-toy-chest/pct#18, the first templates landed for the PCT LA were:

• puppet-module-base project
• puppet class item

See #1 for the actual content.

The puppet-module-base was a significantly stripped down template compared to the original PoC template it was based off.
