Recreating a new credential after creating a credential inline when adding a provider is not allowed

1. Go to Providers > Create; Select a provider; Azure for e.g.
2. Under Cloud Credential; create a new credential
3. The credential would be created regardless of whether it is in/valid.
4. Create a new credential again; the form is loaded with the same values as previously filled form
5. Click on Create button; the form will be constantly loading.

Cloud Credential form values are not reset; and thus user cannot create a new credential in case there was a problem with previously created credential.
