NameError when trying to create a form #8

Attached file shows the procedure for creating a new refinerycms app, installing refinerycms-forms and trying to create the first form. The error message is also in the file (at the bottom of file). I think the most relevant line is this:
/usr/local/rvm/gems/ruby-2.2.8@formsbug2017/gems/refinerycms-core-4.0.1/lib/refinery/generators/named_base.rb:11:in `block in parse_attributes!': uninitialized constant Refinery::Generators::GeneratedAttribute (NameError)
About environment: CentOS 6, Ruby 2.2.8.
