Outcome

Users can deliberately post review decisions from pull request detail without acting on an unexpected pull request revision.

Acceptance

• AC1: GitHub write actions originate only from an explicit user action in pull request detail.
• AC2: Approval and request-changes actions require explicit confirmation and are unavailable from the overview.
• AC3: The target pull request and head are revalidated before posting.
• AC4: A changed target blocks posting and leaves the user able to refresh and reconsider.
• AC5: Background updates cannot redirect an action to another pull request or head.
