Fix formatting issues in scoring.adoc

- Remove trailing line breaks in paragraphs for better rendering
- Remove trailing '+' symbols from table cells
- Fix bullet list formatting (replace tabs with spaces)
- Clean up extra spaces in table headers and cells
- Replace angle brackets with square brackets in placeholders
- Improve overall document consistency while preserving content

These changes ensure the document renders correctly when building
to PDF and HTML formats.
