No header present when using Docker image

I'm creating this Issue because something has changed, and the Header is no longer appearing:

In older versions, there were icons and links along the top-right. Those have disappeared in recent versions.

I'm using the Docker image. I've cleared cookies, recreated the user and database, and tried a different machine. The header appears on first load, and are present if I hit a breakpoint on the first line of javascript, but disappear if I unpause execution.

For reference, here's what it looks like if I put a breakpoint on the first line and reload the page:

Something appears to be causing it to disappear, but I'm not sure what.

Without that menu, I can't log out, switch users, or do much of anything really.
