Breaking changes

• simplex_solve! appears to be working better. Gives a sensible response for unbounded LPs. First basis is still found by brute force; will need to deal with that. Also, the user needs to specify the first basis before solving can begin. Probably should change that.

IMPORTANT: Any version below 0.1.0 is pre-pre-beta.
