Feature Request: Add ability to define custom fields to the PD handler

The customer would like the ability to define custom fields to be populated via tokenization (outside of using the -d flag)

For example:

check_output: "{{.Check.Output}}"
client_url: "https://sensugourl.com/c/~/n/{{.Entity.Namespace}}/events/{{.Entity.Name}}"
client: "{{.Entity.Name}}"

If I understand correctly they want to be able to populate the values for their incidents' custom fields. We need to define an interface to pass those fields. Something similar this might work:

--custom-field-template="check_output={{.Check.Output}}" --custom-field-template=client_url=https://sensugourl.com/c/~/n/{{.Entity.Namespace}}/events/{{.Entity.Name}}

Where the parameter value is split on the = character and the value is applied using templates.

Exact API interface needs to be investigated.
