As a user, I'd like to provide feedback quickly and easily

We have an email address in the hamburger menu in v1.0.1. Suggestions are:

1. add a "text to provide feedback" link somewhere obvious, so that a team member can send a text fast with thoughts on things that are working/not working.
2. add a link to a form in the app that's saved in the database (or sent somewhere)
