Add missing CFP missing fields

There are differences between fields in pybay CFP page and the symposion model ones.

• Do we want the same fields as 2016 or some variation of it?
• There is also the issue of symposion full name and our CFP fname lname. Do we want to implement the later?
• Can we track here what it is needed?
