Add missing US Core Vital Signs profiles

There are missing US Core Profiles that I cut for time.

• DiagnosticReport for Labs (kind of a spiritual subset of DiagnosticReport for Notes but would probably require us partitioning/slicing the DiagReports between the two).
• All the specific Vital Signs profiles (we just have the base Vital Signs checks).

Dan says his priority list would be:

• Blood Pressure
• Height & Weight
