Move Admin Signup to Wufoo / Change notifications

Currently, we're using something other than wufoo for people to sign up to edit their location. We'd like to move this over to this wufoo form:

If for some reason it's a pain in the butt to do this, we mainly need to remove Laura and Bryant and add Sonja to the notification form.
