Document some of the deployment challenges

While @madpah and @bhamail worked on deploying the app, we ran into a number of hurdles around reproducing `.env` and secret settings in `.tf` files, and docker, and other goodies, oh my. Might be worth documenting these, while they are fresh. 

Eventually, we might find an approach that works more simply for dev, CI and deployment cases.
