[Bug]: Closing modified file does not trigger save confirmation

Short summary:
Closing a modified file does not prompt to save changes

What happened?
When I edit a file and then close it (e.g., via tab close), the IDE does not show any prompt to save changes. The file simply closes and the changes are lost without warning.

How can we reproduce it?

1. Open any file in the IDE
2. Make a change (do not save)
3. Close the file/tab
4. Observe that no save prompt appears and changes are discarded

I have the same problem before. Toggle on auto save solved the issue
