update versioning and publishing instructions

to bear in mind

• the current readme instructions do not work
• we need to manually update the version number like this chore(wasm-utils): update version number #12 each time we version/publish
• we should remove guesswork about whether to bump major/minor/patch; whether to push a git tag; etc

A GH action might be a good idea.
