refactor(plugin): rename install to toolkit@stijnvanhulle (#160)

Rename the plugin from 'template' to 'toolkit' so the install reads
'toolkit@stijnvanhulle'. Update the marketplace entry, plugin manifest,
and the install string in README.md, AGENTS.md, and tools/claude/README.md.
