Environment configuration not working

Reported by @ChrisLovering in IRC. It seems like environment configuration is not working (well/at all). Let's write some testcases for this.

887e889 fixed the problem I was having with loading the db url via an env var. I have used a config file for the rest so unsure if this is still a problem for the others.
