fixup smart-health-card-decoder FHIRBUNDLE validation

IPS bundles generated by the HAPI server don't conform to the original SHCD validation rules (specifically that bundle type must be "collection" and that resource ids must be of the form "resource:#"). These rules were implemented to support SHC minimization which isn't relevant for SHLs. Either:

1. (Easy but lame) just set permissive = true in defaults.js
2. (Better) Submit a PR to SHCD that allows disabling of minimization rules.
