Dualsense and Dualshock Controllers randomly lose or don't initialise USB connection

Describe the bug

My PS4 and PS5 controllers randomly disconnect and don't automatically reconnect, or sometimes don't initialise properly on boot when connected through USB. Different cables and different ports still lead to the bug presenting itself. This issue does not seem to affect other USB devices on my computer. Disconnecting then reconnecting the cable on the controller side does not seem to affect the issue. Doing the same on the tower side only works if I wait a few minutes before reconnecting.

What did you expect to happen?

For my controller to successfully connect, then stay connected and working throughout an entire session on my computer.
