Handle XXmg + XXmg dose form

There is another dose form which CALIBERdrugdose does not handle but may be found in clinic medication lists: XXmg + XXmg (e.g. furosemide 40mg + 80mg daily) or XX + XXmg - these should be interpreted the same as '40mg in the morning and 80mg in the afternoon' which CALIBERdrugdose will total to 120mg - not urgent, but maybe on the list of future enhancements.

As part of this may also want to look into separating dose info with link words like "and"

Also requires changes in CDA to express split dosing
