Creating form inside folder, adds the form to root

Issue summary

Creating a folder in Forms, and then create a form inside of that folder. The form is created and add to root, instead of inside the folder. The user then must manually move the form into the appropriate folder.

Steps to reproduce

1. Add a folder to the Forms root
2. Click on that folder and click Create
3. Name the form and click Save
4. The form is then add to root (instead of inside the folder)

Expected result / actual result

Form would be created as a child of the folder selected.
