acc. to me the causes:

• broken or missing db migration, grant_user table might be misconfigured
• wrong permissions on the users table in supabase/postgres
• null ref or foreign key violation when creating the user record
• connection pool timeout during the grant_user transaction

if the auth callback logic is in the oss codebase i can look into it and raise a pr. let me know which file or handler to check.
