chore(release): update CHANGELOG through v4.0.1

Regenerate the tracked changelog with git-cliff. Brings it current for the
already-issued v4.0.1 (which released before the auto-update workflow) and
corrects the stale [Unreleased] section, refiling the starship/Windows work
under [4.0.0] where it was actually tagged.
