csd: add a box to manually exclude {{first article}}

the welcome template is added to most speedy deletion notifications, excluding criteria like vandalism or hoaxes. it'd be useful to have an ability to exclude this (for example, some G11 pages are so blatant it's likely the creator will be immediately blocked, so "Welcome, I hope you stay here" isnt useful
