Add Tracing to the Language Server

Currently, debugging the language server portion of the extension is painful, since we emit no tracing information, and (as stated in #19), most of the time we hit an error, we silently ignore it; not even a log message.

I think we should alter our approach here, and be more liberal with displaying information in the `Output` console.
This isn't something the user will ever see anyways.
