Add request.form data migration support

We are capable of migrating body, cookies, headers, and query params but we also need to be able to migrate forms.

Yeah, I am currently converting an API that was previously JSON-only to multipart/form-data, so support for migrating request.form would be useful for that,

If you dont get a chance to work on it this weekend, I can try to take a look and implement it as well.
