Also fixed up many Library/AMBA_Fabrics/ files to simplify unit testing.
Minor tweaks to doc/How_to_Contribute.{adoc,html}
Uniform AMBA names,
Clean Separation of BSV FIFO-like communication
and AMBA RTL signaling with ready-valid handshake
Rewrote several modules and created new tests.
