[TASK] Refactor "keep on match" behavior for narrative of refined sections

🚧 What needs to be done

Currently, we are only keeping narrative data where the coded data is being refined when there is matching coded data. This behavior needs to be implemented later as an explicit option because it can confuse users who have opted to keep the narrative but don't receive one in the output

🤔 Why it needs to be done

We want to avoid giving users outputs that don't align with what they've configured to avoid confusion

🏗️ Acceptance Criteria

• Refactor behavior to follow user selections (i.e. retain narrative if that's their selection)
• Retain "Keep on match" logic for later to be added as an explicit choice for users for the narrative
