BFD-4255: Add dual enrollment Coverage resource

What Does This PR Do?

• Adds mappings for dual enrollment coverage resources
• Fixes the coverage queries so that beneficiaries with expired coverage will still have their information returned with the status set to "expired"
• Fixes DRG code to be appropriately marked as optional
• Updates the ID properties of some coverage tables to be more accurate - we don't need to include start/end dates in the unique ID for the entity since we're only pulling the most recent record
