Added Office of Historic Preservation to Department Dropdown
The department dropdown has been updated to include the Office of Historic Preservation to ensure accurate submission routing and record processing.
Predefined List Formatting Issue Resolved
Corrected an issue where predefined lists no longer supported spaces, impacting dropdown values shared across multiple forms.
Resolved Index (Zero-Based) Error
Fixed an error preventing certain applications from opening in the UAT environment. Users should now be able to access all impacted apps without interruption.
Submission Date Added to Code Enforcement Appeals View
The Code Enforcement Appeals workflow and its data view now include a submission date field. If missing, the field has been added to the form and surfaced in the view for improved tracking and review.
Lookup Functionality Validated and Restored
Investigated and corrected lookup failures affecting the DND Housing Lottery form and related configurations. Lookup fields are now functioning as expected.

Release Notes

The Office of Police Accountability and Transparency (OPAT) utilizes an AgilePoint form to collect public complaints related to police misconduct. Upon submission and internal updates, automated email notifications are sent to appropriate stakeholders for case tracking and review.
Previously, email notifications related to new and updated complaint cases were being sent to broader distribution groups, this release aligns email notification recipients with OPAT's internal permissions and responsibilities.

The “Graffiti Busters” program provides free graffiti removal services via 311 requests to residents across Boston. The majority of these requests pertain to graffiti on private property, which requires property owners to fill out and return a signed liability waiver.
This release includes updates to the Graffiti Waiver Form submission workflow and its related notification settings to improve communication, clarity, and user experience. In this release the hyperlink labeled “HERE” in the waiver instructions (triggered when the user selects “No” to being the legal property owner) has been updated. Notification recipients for new waiver submissions have been updated.

Release Notes

The “Graffiti Busters” program provides free graffiti removal services via 311 requests to residents across Boston. The majority of these requests pertain to graffiti on private property, which requires property owners to fill out and return a signed liability waiver.

The Office of Police Accountability and Transparency (OPAT) exists to investigate complaints of police misconduct. There is an AgilePoint form for people to report Police misconduct. Add enhancement for OPAT to reassign cases themselves.
This form is used to apply for income-restricted housing through the housing lottery system. This process sends an email notification to the user.

Mass Housing was recently added to the form but not the data download. Add the Mass Housing data to the download.

[LAW - Financial Statement]

Release Notes

The 2024 version of the Annual Statement of Financial Interest form has been reopened in AgilePoint to ensure compliance with Massachusetts Financial Disclosure Law (M.G.L. c. 268B). This law requires all major policy-makers in the City of Boston to submit a financial disclosure form annually. Each year, the Law Department notifies Cabinet Chiefs and Department Heads by email. Anyone under their supervision who holds a major policy-making position must also complete the form. The digital form and associated materials have been updated to reflect the 2024 filing requirements.

This release includes the following updates:

• Refreshed form language for the 2024 filing year
• Updated PDF output file to reflect 2024
• Updated AgilePoint view for internal review and tracking
• Updated Google Drive folders and templates for 2024 document collection
