Indirect vector checks

Add extra checks to indirect_vector insert and emplace methods and expect the index operator and front and back to throw the appropriate exception in tests.
Also do some cleanup like checking in emplace_back if the vector is full, when push pop checks are expected, enabling emplace to be used with const_iterator, utilizing that ipool::create accepts the arguments to create the object.
