docs(skills): sync ce-doc-review and ce-plan with PR #780#782

PR #780 made ce-doc-review default to headless when chained from ce-plan, scoped persona techniques by doc shape (requirements vs plan with Origin), and tightened the adversarial activation trigger. Update the user-facing docs to match: activation criteria, modes table, persona scoping paragraph, and the post-plan flow narrative.

This PR bringsce-doc-review.mdandce-plan.mdin line: refreshed activation criteria, a new paragraph on doc-shape technique scoping, an updated modes table, and a rewritten Quick Example flow matching the headless-then-summary-then-menu pattern.
