feat: read_file failure-rate directive (Closes #1370) - #1377

Track cumulative read_file failures per session. After 4+ failures, inject a _rate_directive telling the agent to stop guessing paths and use search_files/repo_map instead. Addresses the 10.6% read_file failure rate caused by path hallucination. Closes #1370 Co-Authored-By: Hermes Evolution <evolution@hermes.ai>
