docs: refine agent persistence and markdown doc rules - #22

- Persistence: replace the "don't auto-create CONTEXT.md" note with a rule to periodically review .agents/ content and prompt before removing files. - Markdown: scope the style rules to human-facing docs and make CHANGELOG.md an explicit exception to the "describe the now" rule.
