Fix VMM virtual address alignment in cuMemAddressReserve (pass phys_mem_size as alignment to ensure chunk-aligned pool base), fix NIXL transfer agent init/setup, peer overlap detection, and dst split for kv_cache_manager_v2 compatibility.
